Provider and Healing Priory Sign Up

In order to sign up as a provider, health coach, technician etc. within our network or sign up your facility, please fill out the application below and follow all prompts till payment. All applications must be completed, with payment, within 14 days of start date or they will be deleted and you will have to start over. At the end of this application, after payment is submitted, you will see a screen that will tell you your application was completed and sent to Processing. If you do not see this screen your application was NOT submitted. If you have a business with an EIN you must sign up for the 4th option.

It can take up to 30 days for processing. Once you are a provider you will have a yearly renewal fee that must be paid in order to stay compliant with Turtle Island Network. To complete the online forms, you'll need your driver's license, a photo headshot and resume, licenses and/or certifications. IT IS HIGHLY RECOMMENDED TO HAVE THESE ITEMS READY BEFORE BEGINNING THIS APPLICATION.

*Money will be refunded if not approved, w/ exception of $30.00 background check. (Note: Not all fees will be returned) No refunds will be issued after 30 days of approval.
